Dermatology Information Report
It is not uncommon that after I have a speciality clinic appointment, that my Primary Care Physician reviews comments from the clinic with me. Most of the time there are comments that says something like "Patient was well prepared for exam" at the top of the report.
Why? We know our bodies best and see things, over a period of time, not seen or noticed by the doctors. We have to make note of whatever is wrong and tell the doctor what's up.
I have a very bad memory so I write notes and present them to the doctor at the start of the exam. This gives me a record for my file and gives the doctor a starting point for the exam itself.

Please note the following information:
- I have used Dove Soap since 1990 because of dry skin caused by low humidity. I use hand cream several times a day. I have used Vanicream on forearms and small of the back under doctors orders also.
- When I spend extended time outside I use Vanicream Sunscreen (SPF 35)
- I have been diagnosed with Porphyria Cutanea Tarda.
- There are images, on record, showing lesions on my arms and legs.
Please take a look at the following listed items during your exam.
- Non-exfoliating lesions in scalp.
- Brown discoloration on forehead
- “Crusty” like lesions on arms and legs.
- I have pin-point sized sores located on the main trunk and back of my body. Using VA prescribed Vanicream to no avail to reduce the sores.
- Non-exfoliating and exfoliating lesions on both arms.
- Lesions leave white or red color scars.
- Deep scars remain white in color while shallow scars turn brown.
- Imbedded in my hands are small round brown colored cysts.
- “Life Lines” turn into cracks that bubble when hydrogen peroxide is applied.
